Understanding FND — What’s Going On in My Nervous System?

If you’ve been told you have Functional Neurological Disorder (FND), you may have felt confused, worried, or even dismissed. Many people hear things like “your scans are normal” or “there’s no damage” and wonder, “Then why do my symptoms feel so real?”


Let’s start with the most important message:


FND symptoms are real, common, and treatable. They are not imagined, exaggerated, or “all in your head.”


This post is meant to explain FND in a clear and reassuring way, so you can better understand what’s happening in your nervous system and how physical therapy can help.



The Nervous System: Hardware vs. Software

One helpful way to understand FND is to think about the nervous system like a computer.

  • The hardware is the physical structure: the brain, spinal cord, and nerves.

  • The software is how the brain sends, receives, and organizes signals, like with movement, sensation, balance, speech, and automatic actions.

In many neurological conditions (like stroke or multiple sclerosis), the hardware is damaged. This damage shows up clearly on scans.

In FND, the hardware is intact—but the software isn’t running smoothly.

The brain is having trouble sending or organizing signals correctly, even though the structures themselves are healthy. This miscommunication can lead to very real symptoms such as:

  • Weakness or paralysis

  • Tremors or abnormal movements

  • Difficulty walking or balancing

  • Changes in sensation

  • Speech or coordination problems

Nothing is “broken,” but the system isn’t working the way it should.


Why Symptoms Can Be So Strong

One of the most confusing parts of FND is how intense symptoms can be despite normal test results.

That’s because the nervous system controls everything, including movement, sensation, reflexes, and automatic patterns we usually don’t think about. When signaling goes off track, the effects can feel sudden, unpredictable, and overwhelming.

Here’s an example:

Imagine your phone’s screen freezes. The phone isn’t broken—you didn’t drop it, and the battery works—but tapping the screen doesn’t lead to the right response. You might press harder, tap faster, or restart the app. The problem isn’t the phone itself; it’s how the system is responding at that moment.

FND works in a similar way. The brain may send mixed or incomplete signals, or attention may become “stuck” on certain movements or sensations, making them harder to control.



If There’s No Damage, Why Did This Happen?

This is a very common and very valid question.

FND does not have one single cause. It often develops from a combination of factors that affect how the nervous system functions, such as:

  • Physical injury or illness

  • Stress on the body (like fatigue, pain, or concussion)

  • Emotional stress or anxiety

  • Long periods of focusing on symptoms or movement

  • Previous neurological events

Importantly, you do not have to have a history of trauma or mental health problems to develop FND. Many people with FND are high-functioning, motivated, and physically active before symptoms begin.

Think of it like a traffic jam in the brain. One disruption can lead to a backup that continues even after the original trigger is gone.



Why Being Told “Nothing Is Wrong” Feels So Wrong

When people hear “your MRI is normal,” it can feel invalidating, especially when symptoms are affecting school, sports, work, or daily life.

A more accurate way to say it is:

“There is something wrong with how the nervous system is functioning, even though the structure looks normal.”

That distinction matters. It explains why symptoms are real and why recovery is possible.



The Good News: The Nervous System Can Learn Again

One of the most hopeful things about FND is that the nervous system is changeable. The brain is constantly learning and adapting. This is known as a process called neuroplasticity.

Physical therapy for FND is not about strengthening damaged muscles or fixing broken nerves. Instead, it focuses on:

  • Retraining normal movement patterns

  • Reducing over-control and over-attention to movement

  • Helping movements become automatic again

  • Rebuilding confidence and trust in the body

Because the hardware is intact, the brain can learn new (and more efficient) ways to send signals.

  • Your symptoms are real and not imagined

  • FND is a disorder of function, not damage

  • Normal tests do not mean “nothing is wrong”

  • Recovery focuses on retraining, not repairing

  • Improvement is possible—even if symptoms have lasted a long time
